Kronik migreni olan çocuk ve ergenlerde depresyon daha sıktır

Abstract

Amaç: Çocuklar ve ergenler, özellikle de kronik migreni olanlar, yaşam kalitesi ve okula devam
açısından daha kötü sonuçlara sahiptir. Migreni olmayan çocuklarla karşılaştırıldığında, anksiyete
ve depresyon gibi duygudurum bozuklukları yaşama olasılıkları daha yüksektir. Migren, anksiyete
ve depresyon gibi bozukluklarla ilişkisi üzerine en çok çalışma yapılan ağrı bozukluğudur. Ancak
epizodik migren ve kronik migrende bu komorbiditelerin sıklığını karşılaştıran yeterli çalışma
yoktur. Yöntem: Kronik migren tanısı olan 75 hasta ile epizodik migren tanısı olan 150 hasta
olmak üzere toplam 225 çocuk ve ergen baş ağrısı hastası çalışmaya dahil edildi. Veri seti Türkiye
baş ağrısı veri tabanı Mersin dalı kullanılarak oluşturuldu. Hastalar ağrı özellikleri ve eşlik eden
psikiyatrik komorbid hastalıklar açısından geriye dönük olarak değerlendirildi. Bulgular:
Beklendiği gibi ağrı şiddeti ve atak süresi kronik migreni olan hastalarda daha yüksekti (sırasıyla
p<0.001 ve p=0.027). Kronik migren grubunda depresyon oranları epizodik migren grubuna göre
daha yüksek iken, dikkat eksikliği hiperaktivite bozukluğu ise epizodik migren grubunda
istatistiksel olarak daha yüksekti (p=0.001). Anksiyete ve obsesif-kompulsif bozukluk açısından
ise gruplar arasında istatistiksel olarak anlamlı fark yoktu (p>0.05). Sonuç: Migreni olan çocuk ve
ergenlerde günlük yaşam aktiviteleri ve okul başarısı önemli ölçüde etkilenmektedir. Eşlik eden
psikiyatrik komorbiditeler de bu duruma katkıda bulunmaktadır. Bütünsel yaklaşım ve tedaviyi
optimize etmek için bu komorbiditelerin baş ağrısı bozukluklarıyla birlikte yönetilmesi önemlidir.

Depression is more common in children and adolescents with chronic migraine

Abstract

Aim: Children and adolescents, particularly those with chronic migraine, have worse outcomes
regarding quality of life and school attendance. Compared with children without migraine, they
are more likely to have anxiety and mood disorders, such as depression. Migraine is the most
studied pain disorder in terms of being associated with disorders such as anxiety and depression.
However, there are not enough studies comparing the frequency of these comorbidities in
episodic migraine and chronic migraine. Method: A total of 225 pediatric headache patients, 75
with chronic migraine and 150 with episodic migraine were included. The data set was composed
using the Turkish headache database, Mersin Branch. The patients were evaluated in terms of pain
characteristics and accompanying psychiatric comorbidities retrospectively. Results: As
expected, pain intensity and duration of attacks were higher in patients with chronic migraine
(p<0.001 and p=0.027, respectively). While depression rates were higher in the chronic migraine
group than in the episodic migraine group, attention-deficit hyperactivity disorder was
statistically higher in the episodic migraine group (p=0.001). When compared in terms of anxiety
and obsessive-compulsive disorder, there was no statistically significant difference (p>0.05).
Conclusion: Daily living activities and school success are significantly affected in the pediatric
migraine. Concomitant psychiatric comorbidities also contribute to this condition. It is important
to manage these comorbidities together with headache disorders in order to optimize the holistic
approach and treatment.

In addition to research articles, scientific articles such as reviews, case reports and letters to the editor are published in the journal. Our journal, which has been published via e-mail since its inception, has been published both online and in print. Following the Participation Agreement signed with TÜBİTAK-ULAKBİM Dergi Park in April 2015, it has started to accept and evaluate online publications.
